Transition Planning
A transition plan is your opportunity to plan your future. Figuring out what you are passionate about, what are you interested in & how can we turn that passion in a possible contribution to your community. Transition planning begins as early as age 14, or when an individual enters secondary school programming.
For individuals who require additional supports after high school, there is a very specific government timelines for the application process to acquire those supports by the time the student turns 18, I can help coach you through the process.
For individuals who are not in a traditional school environment -figuring out what learning might need to occur to enter a post secondary education program & have completed all, if not more than the minimum pre-requisites.
